STEP 1: Purchase any one [1] promotionally marked HEAVEN ice-cream product between 31/01/08 and 08/11/08.

STEP 2: Download and print the relevant claim form for your purchase of a promotionally marked HEAVEN ice-cream.

SINGLE PACK CLAIM FORM

MULTI-PACK CLAIM FORM
To make a claim for the purchase of a multi-pack of HEAVEN promotionally marked product (multiple HEAVEN products in a pack of 4) click here.

STEP 3: Fill in all fields on the claim form in full (including an answer in no less than 15 words as to why you did not think HEAVEN tastes the best).

STEP 4: Attach the form of proof of purchase requested on the applicable claim form relevant to your purchase of promotionally marked HEAVEN product.

STEP 5: Mail in the completed claim form with the requested proof of purchase to the address indicated on the claim form.
